West and colleagues highlight a critical limitation of weight loss drugs: rapid weight regain and reversal of cardiometabolic benefits after discontinuing treatment.1 Although clinically important, these findings overlook exercise as a key determinant of outcomes after treatment, offering mechanistic and functional benefits that pharmacotherapy is not able to replicate.
