Background Sentinel Stroke National Audit Programme(SSNAP) data indicated our centre was performing at below national thrombolysis rates (9.5% compared to national average of 11.4%) at time of audit registration.Aim To identify gaps in performance and potential areas for improvement by a deep dive auditMethods We audited consecutive patients presenting to Sheffield (HASU unit RHH) from 1st February to 1st May 2024, against national standards:100% of acute stroke patients should have NIHSS on arrival; should be scanned within 1 hour from arrival; and all acute stroke patients arriving within 4.5 hours of onset should be offered thrombolysis if no clear contraindication.Patient notes were reviewed, data anonymised and recorded on EXCEL, and process times evaluated.Results 100% (33/33) of thrombolysed patients were scanned within 60 minutes.81.2% (26/33) were scanned within 20 minutes.62.5% (20/33) received thrombolysis within 60 minutes of arrival.NIHSS < 4: 12 patients (37.5%) treated. NIHSS ≥ 15: 27 patients (84.3%) treated.Door-to-scan 12.4 minutes (average). Door-to-needle: 68.1 minutes (55% ≤ 60 minutes).Our action plan included educational initiatives, and EPR system upgrades aim to enhance SSNAP data accuracy, treatment rates, and patient outcomes.These initiatives improved thrombolysis rates to 13.3%, exceeding the national average (August–November 2024) ramya.satyadi@nhs.net
