Advances in congenital and acquired heart disease management have allowed more women than ever to reach reproductive age and pursue pregnancy. Yet, as maternal cardiology evolves, new stressors emerge that challenge conventional risk stratification.1 Among them, twin pregnancy represents a uniquely high circulatory challenge that pushes maternal cardiac reserve to its limits. Until now, data have been scarce on its impact in women with structural heart disease.
