We read with great interest the trial by Virtos et al comparing Wide-Awake Local Anesthesia No Tourniquet (WALANT) with ultrasound-guided axillary block (USGAB) for outpatient endoscopic carpal tunnel release (eCTR),1 which provides robust evidence that WALANT is non-inferior to USGAB for eCTR, with notable benefits for patient recovery. The authors are to be congratulated for conducting the first double-blind trial on this topic2 in a setting where blinding can be challenging, and for addressing a clinically relevant question as the use of WALANT continues to expand.
