A 38-year-old man had a 3-year history of progressively impaired balance with difficulty walking. He was unsteady on his feet, especially with his eyes closed, and his gait had become more broad-based. He had altered sensation in his feet but no weakness. Concerned that he would fall, he took precautions and adjusted his daily routine; for example, having baths instead of showers. He was otherwise well apart from anxiety and took sertraline 100 mg daily. He was a non-smoker and drank occasional alcohol, although until 5 years before he had consumed up to 50 units weekly. There was no history of illicit drug use. His birth and developmental history had been normal.
