A male patient in his early 60s presented with a 1-week history of acute confusion and a 1-day history of facial twitching. A collateral history was attained from his family, stating he was functionally independent with no medical history of cognitive issues. There was no report of trauma or recent foreign travel. His initial blood results, arterial blood gas and neurological examination were normal, with no indication of meningism. Additionally, CT of the head did not reveal any intracranial abnormalities.
