Introduction Lattice is loaded on an innovative mechanical balloon delivery system. With the help of the self-expanding mechanical balloons, Lattice implant is easy to expand and open while unsheathing. To evaluate the safety and efficacy of Lattice flow diverters (FDs) for the treatment of the aneurysms beyond the circle of Willis.Case Description We retrospectively reviewed the prospectively maintained database in our center and enrolled patients with aneurysms beyond the circle of Willis that were treated with Lattice FDs. Between May 2024 to March 2025, 18 patients with 18 aneurysms met the inclusion criteria and were included in this study, with a median age of 60.8 years old. 18 Lattice FDs were deployed. Perioperative complications were noticed in 1 patients (5.5%). All patients received clinical follow-up, with the mortality and long-term morbidity of 0 and 0, respectively. Angiographic follow-up data were available for 16 patients with an interval of 9 ± 2 months. Ten patients showed complete or nearly complete occlusion of aneurysms (OKM grading scale D and C); 2 cases were revealed incomplete occlusion (OKM grading scale B), and 4 cases remained unchanged (OKM grading scale A). The existence of the perforators derived from aneurysms was associated with a lower occlusion rate.Conclusions Lattice flow diverters are reliable in the treatment of distal aneurysms with a high technical success rate and low permanent disability rate.Conflict of Interest Yes Lattice flow diverters
