Intrahepatic cholangiocarcinoma (iCCA) invasion into the right atrium (RA) is a rare presentation and represents an unfavourable prognosis. We report a case of a large intracardiac mass occupying the RA in a patient with newly diagnosed iCCA. The case highlights the importance of multimodality imaging in the diagnosis and management of this rare presentation of iCCA.
