Background Craniofacial Microsomia (CFM) is a congenital condition affecting 1:3000-5000; characterised by variable underdevelopment of the facial structures. At GOSH, children with CFM receive multidisciplinary care from oral and maxillofacial, plastic surgery, orthodontics and paediatric dentistry. To improve clinical records, a dedicated records clinic established in February 2024 ran concurrently to the CFM multidisciplinary clinic. This was to ensure standardised internationally agreed patient centred outcome measures, intraoral scans, clinical photographs and patient priorities of care were recorded This intends to ensure patient concerns are addressed during appointments, allow growth monitoring, facilitate audit/research and international outcome comparison to improve clinical outcomes whilst reducing duplicate appointments.Aims This Quality Improvement Programme (QIP) ensured all patients attending the CFM clinic completed:ICHOM measures (ages 2, 5, 8, 12, 22)3D intraoral scan (≥6 years)Clinical photographsPatient priorities questionnaireMethodology After each clinic from March 2024 to July 2025, one examiner (AH) assessed records to ensure correct documentation was collected. Findings were logged and discussed with the supervising consultant (SM), enabling timely improvements in the process before subsequent clinics.Results Baseline compliance in March 2024 was 49%, performance improved to 97% by July 2025. A temporary reduction occurred in February 2025 (87%) due to staff changeover. Consistent compliance was highest when the same clinician oversaw the clinic, highlighting the importance of continuity despite staff changes.Conclusion This QIP substantially improved record-keeping for CFM patients. Sustained change was achieved through a continuously updated SOP, standardised note templates, use of iPads for data collection, and active engagement of staff to optimise bookings and workflows. Assigning a responsible clinician per clinic further embedded the process to support consistent high-quality care.
