The Trainee Collaborative for Research and Audit in Hepatology UK (ToRcH-UK) has successfully transitioned from its inception into a trainee research network (TRN) delivering multiple national projects. Our first project led to the development of an updated British Society of Gastroenterology (BSG)/British Association for the Study of the Liver (BASL) admission care bundle for decompensated cirrhosis and tangible improvements to services. 1 2 Responding to a call in 2023, our second project SHiNE-UK involved a national evaluation of hepatocellular carcinoma surveillance, routes to diagnosis and treatment utilisation; generating two large and novel cohorts.3 The inherited model was project focused, trainee-led and informal—aiming to deliver projects that address important clinical problems and improve outcomes for patients with liver disease.4 While this approach effectively empowers trainees and allows us to remain nimble, it also creates challenges when scaling to support multiple projects over time.
