068 Leveraging clinical pearls and evidence-based physical exam to prevent overtesting and overdiagnosis

Patients often present with a variety of clinical symptoms or laboratory abnormalities which lead to extensive unnecessary testing resulting in overdiagnosis, anxiety, and expense. In many cases, a knowledge of various clinical pearls will allow the clinician to confidently arrive at an accurate diagnosis at the bedside. Clinical pearls are pieces of clinically relevant knowledge based on experience or observation. Using clinical pearls in addition to evidence-based medicine physical examination maneuvers and an understanding of laboratory testing results can avoid excessive and unnecessary testing. This requires a systematic approach to the integration of the clinical pearls in the clinical encounter and the ability to communicate effectively with the patient to allay anxiety. The goal of this approach is to achieve high value care.This interactive workshop will be lead by three faculty from the Mayo Clinic; while best known as a tertiary care center, it also provides primary care to a local population of over 140,000 patients. In aggregate, the three faculty have over a century of both outpatient and inpatient primary care and have intentionally collected high-yield clinical pearls for many years.The workshop format will include a series of brief clinical scenarios highlighting common conditions where clinical pearls can provide information that allows the clinician to avoid unnecessary testing. Throughout the scenarios, participants will provide input, learn the evidence-based physical examination maneuvers, share experiences including challenges with similar situations, and discuss best practices.Previously, the faculty have successfully engaged participants through questions and shared experiences, ensuring that learning the content of clinical pearls and their application was achieved.
