A 24-year-old man presented with a 7-year history of recurrent melena, acutely worsening 3 days prior. He had undergone endoscopic band ligation and glue sclerotherapy for gastro-oesophageal varices, yet intermittent bleeding persisted without advanced aetiological evaluation beyond cirrhosis/thrombosis screening.
