An 8-year-old girl with refractory epilepsy returns to the paediatric neurology clinic following the initiation of cannabidiol (CBD) treatment, after prior trials of multiple antiepileptic drugs, ketogenic diet and a vagal nerve stimulator (VNS) proved ineffective. While her parents report a slight reduction in seizure frequency, there has been no notable improvement in her quality of life (QoL). Subsequently, a 9-year-old girl with a similar clinical history, including refractory epilepsy, a ketogenic diet, VNS and recent CBD initiation, presents with comparable results: modest seizure control but no significant improvement in daily functioning. These cases lead the neurologist to consider that, for complex patients such as these, the primary objective extends beyond seizure control to encompass improvements in overall QoL.
