Managing geriatric patients on antiplatelet therapy after a fall presents a significant clinical challenge. Although research has often focused on aspirin,1 the risks associated with P2Y12 inhibitors or dual antiplatelet therapy (DAPT) remain less clear. A pivotal study by Wang et al now offers insights into this dilemma. In their prospective study of 3709 patients, Wang et al present a landmark finding: although preinjury use of aspirin and DAPT is associated with increased odds of intracranial hemorrhage (ICH), use of Adenosine diphosphate receptor inhibitor (P2Y12) inhibitors such as clopidogrel are not.2
