The 2025 consensus practice guidelines on sacroiliac joint (SIJ) complex pain 1 provide comprehensive, evidence-based recommendations for identifying, diagnosing, and treating SIJ-mediated pain. The guidelines were developed through collaboration among 27 professional academies or societies and address 21 key clinical questions covering epidemiologic, diagnostic, interventional, and surgical aspects of SIJ pain management. Recommendations were developed using a modified Delphi consensus process, with ≥75% agreement required for consensus, and graded using the US Preventive Services Task Force evidence levels.
