Introduction From an early age, gymnastics requires a high level of technical skill, along with mental and physical strength; however, a high injury rate constitutes a considerable challenge, particularly for young gymnasts. This study aimed to describe the distribution of clinical findings, including apophyseal tender spots, in TeamGym gymnasts aged 10–30 years with prolonged lower-extremity overuse injuries.Materials and Methods Over a 10-month season (August 2021–June 2022), a cohort of 519 gymnasts were prospectively monitored weekly via text messages regarding injury status, training, and competition exposure. All reported injuries were verified and classified as acute or overuse through structured telephone interviews. Gymnasts with prolonged lower-extremity overuse injuries (injury lasting ≥ 3 consecutive weeks) were invited to undergo a clinical examination based on a prespecified protocol.Results Of 519 gymnasts, 174 (33.5%; mean age 14.4 years) had 306 prolonged overuse injuries, resulting in 450 clinical findings. Among males, 49 of 149 (32.9%) were affected; among females, 125 of 370 (33.8%). Of the 450 clinical findings, 165 involved apophyseal tenderness (36.7%) and 285 (63.3%) did not. Apophyseal tenderness most frequently involved the knee (66.1%), followed by hip/pelvis (23.6%) and foot/ankle (10.3%), whereas other clinical findings most frequently involved patellofemoral pain syndrome (15.1%), medial tibial stress syndrome (11.2%), and anterior ankle impingement (10.2%).Conclusions One-third of prolonged overuse injuries were associated with apophyseal tenderness, predominantly located at the knee, followed by the hip/pelvis and foot/ankle regions. Other clinical findings included patellofemoral pain syndrome, medial tibial stress syndrome, and anterior ankle impingement.
