Morley and colleagues warn that artificial intelligence (AI) driven NHS reform risks compounding health inequalities if digital exclusion, algorithmic bias, and individualised prevention remain untackled.1 The more fundamental problem is the system misunderstanding care itself, falsely assuming that technology can substitute compassion and trust. Care is a relational, emotional, and contextual act. Yet policy continues to treat it as an engineering problem, replacing presence with prediction and relationships with risk scoring. AI becomes the latest in a long line of “transformation” tools promising efficiency, while neglecting the moral and affective labour of care. It is NHS managements’ true blind spot.
