A 70–80 year-old man with ischaemic cardiomyopathy, advanced heart failure and chronic kidney disease is admitted with worsening shortness of breath. Over several days, his kidney function deteriorated further and he became delirious. The intensive care unit team believes even aggressive interventions—intubation, dialysis and vasopressors—would offer little benefit. Cardiopulmonary resuscitation (CPR) would be non-beneficial. His family, overwhelmed and uncertain, must now make life-altering decisions on his behalf, unsure what he would truly want.
