When contact lenses harbour two bacillus species: managing a rare polymicrobial keratitis

We present a case of a male patient in his late 40s who visited our emergency department with a 24-hour history of photophobia, severe ocular pain and epiphora in the right eye after 29 days and nights of continuous soft contact lens wear. Best corrected visual acuity (BCVA) was 0.00 log MAR bilaterally. Slit-lamp biomicroscopy revealed mild conjunctival hyperaemia, a paracentral corneal ulcer with stromal haze and a faint epithelial halo ( figure 1A, B). A corneal scrape was obtained from the lesion using a sterile 21-gauge needle at the slit lamp at presentation for microbiological analysis, in accordance with standard care.
