A255 Stenting of intracranial arteries during endovascular ischemic stroke treatment

Introduction Intracranial stenting is a potential rescue strategy after failed thrombectomy.Aim of Study To evaluate the procedural and clinical outcomes of patients who underwent acute stenting of intracranial artery during endovascular treatment of ischemic stroke at our institution.Method A single-center, retrospective, non-randomized study including 34 patients (median age 72,5 years; 44% male) treated between 2021 and 2024 was conducted. Imaging and clinical follow-up data from patients treated with intracranial stenting due to atherosclerosis 91,2% (n=31) or disection 8,8% (n=3) were collected. Periprocedural antiplatelet therapy was selected based on drug availability, with eptifibatide or cangrelor being used in most cases.The group was divided into patients with anterior and posterior circulation stroke, and the data were compared.Results Out of a total of 573 patients, stenting was performed in 21.5% (n=123), with intracranial stenting accounting for 5.9% (n=34). In total, 41 stents were implanted in 36 vessels. Concurrent stenting of the extracranial segment was required in 20.5% (n=7). Thrombectomy preceded stenting in 85.3% (n=29) of cases. A good clinical outcome (mRS 0–3) at 3 months was achieved in 41.2% (n=14). The 3-month mortality rate was 32% (n=11), with 20.8% (n=5 of 24) in the anterior circulation group and 60% (n=6 of 10) in the posterior circulation group. The difference was statistically significant (p=0.047).Conclusion Rescue stenting appears to be an effective treatment option in cases of persistent intracranial artery occlusion, even under challenging clinical and technical conditions. Its benefit in terms of clinical improvement and mortality reduction is more evident in anterior circulation strokes.Conflict of Interest No
