We read with great interest this important case report and literature review describing the catastrophic consequences of accidental intradural chlorhexidine injection. This report makes a major contribution to patient safety in neuraxial anesthesia and rightly reminds the community that such errors, although rare, may result in devastating and sometimes fatal neurological injury.1
