A 71-year-old man presented to the emergency department with sudden onset sharp left-sided chest pain following a severe coughing episode. He was an ex-smoker with a background of chronic obstructive pulmonary disease, obesity (body mass index 31), type 2 diabetes and an umbilical hernia.
