PO:13:342 Do diet habits influence the quality of life of patients with systemic lupus erythematosus? Results from baseline clinical data of the multi-omic multi-institutional SMILES study

Objectives SMILES is a multicenter Spanish endeavor focused on a comprehensive molecular characterization of a cohort of lupus patients using transcriptomic, single cell sequencing and microbiome analysis. In this report we used baseline clinical data about diet habits to analyze potential associations with quality of life (QoL), disease activity and accrued damage.Methods Forty-one Rheumatology departments across Spain participated. We enrolled adults (>=18 years) fulfilling 1997 or 2019 ACR classification criteria for SLE. Evaluation included: medical record review, clinical assessment, blood, urine, fecal and cutaneous microbioma samples and cutaneous biopsies. SLEDAI 2K and BILAG indices for activity evaluation and SLICC disease index (SDI) for damage were used. An ad hoc diet habits questionnaire was used to assess this item with questions about the frequency of consuming fish, meat, legumes, bread, eggs, pasta, sweets and cold cuts using four-option responses about of the days of consumption per week (0, 1-2, 3-5 and 6-7). Additionally, other variable, pro-inflammatory diet (PID), was created for those patients with top consumption of sweets and cold cuts. QoL was measured with the LupusQoL across eight domains: physical health, pain, planning, intimate relationships, burden to others, emotional health, body image, and fatigue. Linear regression models were used in theses analyses setting a p value =<0.05 as statistically significant.Results 220 patients (90% women; mean age 45.8 ± 12.6 years; mean disease duration 12.0 ± 9.8 years) were included. PID was not associated with any QoL domain, clinical activity or damage. Moreover, the individual analysis of the different types of food did not give any relevant association, with the exception of egg consumption which showed worse results in the following QoL domains: body, burden, emotional, fatigue, pain and physical (p =<0.05), fish and legumes consumption associated with pain QoL domain and higher SLEDAI (p =<0.05) and diary consumption associated with fatigue (p= 0.02).Conclusions Overall, no relevant associations were found between diet habits and QoL, clinical activity and damage. The worse outcomes observed with greater intake of eggs and with fish, legumes and diary consumptions have not clear explanations and require further investigations.
