An older man in his 80s presented to a referring hospital after sustaining severe blunt chest wall trauma after being trampled by a horse. He was hypotensive on presentation but responded appropriately to two units of packed red blood cells. Chest X-ray revealed a right-sided hemopneumothorax, which was treated with a thoracostomy tube.
