A man in his late 40s with no significant medical history was admitted after 3 weeks of progressive confusion, anterograde amnesia, misidentification of family members and recurrent panic episodes. On admission, he was disoriented to time and place, with a Montreal Cognitive Assessment (MoCA) score of 14/30 and Mini-Mental State Examination (MMSE) score of 15/30, showing marked impairment in recall and attention.
