An infant (weight 8.5 kg, height 70 cm) presented with 7 days of high-grade fever and reduced oral intake. There was no rash, conjunctival injection, oral mucosal change, extremity change or cervical lymphadenopathy. Laboratory investigations demonstrated leucocytosis, haemoglobin 9.5 g/dL, C-reactive protein 83 mg/L and thrombocytosis (1286×10³/µL). Despite empirical antibiotic therapy, fever persisted and no infective focus was identified.
