An elderly female in her late 60s came to the medical emergency department with complaints of altered sensorium, aphasia and generalised weakness for 2 days. Her medical history was significant for diabetes mellitus and coronary artery disease. There was no history of prolonged fasting, any recent diuretic use, significant gastrointestinal or urinary fluid loss. No relevant family history was noted.
