LB3 The new challenge in neurointerventions: recanalization of intradural chronic total occlusions (idCTO) – the re-CHILL study

Introduction Chronic total occlusions (CTOs) have been successfully targeted in various vascular territories over recent years. Within the neurovascular field, many centers advanced the recanalisation of CTOs involving cervical ICA occlusions, enabling rapid reperfusion and eliminating the need for bypass surgery. Pioneering centers are extending these efforts further by tackling CTOs extending into intradural arterial segments or those entirely intradurally located. However, reasons for treatment reluctance in these cases are the potential periprocedural complications, especially the risk of intradural dissection, vessel perforation and embolic events.Aim We evaluated the endovascular techniques, feasibility, safety, and clinical outcomes of endovascular treatment for symptomatic intradural CTOs.Methods The Re-CHILL Study is a retrospective international multicenter project (Europe, USA, Asia) collecting cases of EVT for intradural CTOs. Inclusion criteria target adult patients with symptomatic intradural CTOs (ICA, MCA, vertebral and basilar artery). Primary endpoints included technical success, and functional outcomes, measured by the modified Rankin Scale at 3 months and the last follow-up. Safety endpoints include complication rates, new ischemic strokes, and overall mortality.Results We identified 10 international center contributing to the study. The data collection will be completed by the end of August. This first interim analysis of collected cases will mainly focus on the techniques used and immediate periprocedural complications.Conclusion The Re-CHILL is the first retrospective data collection on the intradural CTOs to elucidate the technical feasibility and safety of endovascular recanalisation and to establish endovascular treatment strategies for these patients.
