Introduction Low-value care (LVC) refers to interventions that provide little or no benefit to patients and have the potential to cause harm or incur unnecessary costs, thereby wasting the limited resources of the healthcare system. Recently, an agreement was reached among departments within our institution to limit the use of endocrinological LVC, which includes: vitamin D testing and supplementation in healthy individuals, bone health-related studies (such as bone density scans for osteoporosis screening in individuals under 60 years of age, repeat bone density scans within two years in patients with primary osteoporosis, and intensive evaluation of calcium-phosphate metabolism in women with primary osteoporosis), and thyroid-related studies (such as thyroid ultrasound or TSH, T4, and T3 testing as part of routine follow-up in hypothyroid patients). It is necessary to assess the frequency of these LVC practices to establish a situational diagnosis.Objectives To describe temporal trends in the use of endocrinological LVC within our institution.Methods A retrospective cohort study was conducted using data from electronic health records (EHR). Adult patients enrolled in our institution’s health insurance between January 2014 and August 2023 were included. The number of tests performed per year was evaluated and divided by the number of active enrollees to calculate usage rates and compare them over time.Main Results A total of 246,788 patients were included, with a median follow-up of 6 years. Of these, 9.6% were hypothyroid.Vitamin D testing increased over time. In 2014, 13% of enrollees had undergone testing at least once in the previous year, a proportion that rose to 22% in 2023. Testing was more common among women and older adults.Regarding bone health-related studies, approximately 15% of women underwent a bone density scan between the ages of 50 and 60. Among women with primary osteoporosis, around 12% underwent intensive evaluation of calcium-phosphate metabolism. Additionally, 74% of bone density scans were repeated within two years. These trends remained stable over time.For thyroid-related studies, the proportion of patients undergoing TSH+T3+T4 (total or free) testing decreased from 50% to 38% between 2014 and 2022. Similarly, the proportion of patients undergoing thyroid ultrasound decreased from 14% to 8% during the same period.Discussion and Conclusions LVC is prevalent within our institution. This information is relevant for planning de-implementation interventions.
