Autologous conditioned plasma is not platelet-rich plasma

We read with great interest the study “Intra-articular corticosteroid injections versus platelet-rich plasma as a treatment for cervical facetogenic pain: a randomized clinical trial”1 and commend the authors for investigating innovative, non-steroid interventions for the treatment of cervical spine pain. However, we wish to raise an important consideration regarding the authors’ use of the term “platelet-rich plasma” to describe their intervention, which utilized autologous conditioned plasma (ACP). ACP typically contains platelet concentrations only marginally above the patient’s baseline whole blood platelet count. The authors correctly note that previous studies of ACP generally report a platelet concentration approximately 1.3 times baseline. Although platelet concentrations were not measured in the study by Allison et al, they were likely similar. ACP has been previously studied in randomized clinical trials of knee and ankle osteoarthritis, where it has failed to provide significant advantages over placebo or hyaluronic acid injections.2 3
