Subcapsular hepatic haematoma in a postpartum woman with severe pre-eclampsia

Subcapsular hepatic haematoma (SHH) is an uncommon but potentially life-threatening complication of hypertensive disorders of pregnancy, particularly Hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome and severe pre-eclampsia. We report the case of a woman in her 30s who developed a large SHH without rupture in the postpartum period following severe pre-eclampsia. Early imaging and conservative management strategy led to favourable maternal and neonatal outcomes. This case highlights the importance of maintaining a high clinical suspicion, timely imaging and multidisciplinary collaboration to optimise prognosis.
