6.2. Playing the long game: potential for extending corneal cold storage time

Purpose To compare endothelial cell density and morphometry, thickness, and transparency of corneas stored in XTRA·4 v. Optisol-GS for up to 4-weeks.Methods Mate corneas were recovered into XTRA·4 or Optisol-GS (20 pairs) and stored at 2–8°C for 26 days. ECD, HEX, and CV were evaluated by specular microscopy. Central stromal thickness (CST) was examined by OCT. Endothelium, stromal edema, and Descemet folds were examined by slit lamp using defined grading scales.Results Average ECD, CV, and HEX for the XTRA·4 (2558±324, 32±5, 61±6) and Optisol-GS (2509±368, 34±4, 60±4) groups were similar through day 21 (P > 0.05 for all). Hexagonality was difference between the two cohorts at day 26 (XTRA·4: 56±4 v. Optisol-GS: 52±4, P < 0.05). Stromal thickness measured at initial evaluation for corneas in XTRA·4 was 520±38 and 502±28 those in Optisol-GS (P>0.05). The thickness of corneas stored in XTRA·4 remained the same for 28 days and was not significantly different to those stored in Optisol-GS on day 7 (P>0.05), but was significantly thinner on days 14, 21, and 26 (P<0.05). Overall endothelium quality assessed by slit-lamp remained similar between the two cohorts. Stromal edema and Descemet folds for corneas stored in XTRA·4 remained constant, while mate corneas stored in Optisol-GS increased by two grades during storage.Conclusions Corneas preserved in XTRA·4 exhibit characteristics that are at least equivalent, and in some cases superior, to those stored in Optisol-GS. The tissue integrity of corneas maintained in XTRA·4 remained stable throughout the storage period, offering potential to streamline tissue-surgery coordination.
