Introduction The National Endoscopy Database (NED) collects real-time data from electronic endoscopy reports across the United Kingdom. The latest iteration (NEDi2.1) collects faecal immunochemical test (FIT) results. The NHS England Cancer Programme has sought a national solution to collect FIT data to ease the burden on Cancer Alliances (CAs). We present the capabilities of NED in fulfilling this role for all colonoscopies performed primarily because of patient symptoms.Methodology All colonoscopies in England uploaded to NEDi2.1 from January-June 2025 were extracted. Procedures were labelled as one of therapeutic, bowel cancer screening, abnormal prior investigation, surveillance, inflammatory bowel disease, symptomatic or only FIT recorded using previously reported methodology. All non-symptomatic procedures were excluded.FIT results were categorised into bands (<10, 10-100, >100, FIT positive/negative qualitative, FIT performed but no result, or FIT not performed). A primary metric of ‘the percentage of symptomatic colonoscopies informed by a FIT >10ugHb/g’ (recommended target 85%) was proposed. Performance on NED was compared to CA dashboards at a regional level for the same period.Results 70.1% (n=89) of NHS services in England were uploading to NEDi2.1 in the time-period. 80% of regions (n=16) showed FIT rates on NED that were comparable (within 10%) of those on the CA dashboards. Table 1 shows the proportion of procedures in each FIT band. In NHS organisations, 44.6% of procedures (95% CI 44.3-44.9) were informed by a FIT >10ugHb/g (i.e. FIT ‘positive’). 41.9% (41.6-42.2) of procedures were labelled as FIT not done vs 11.0% from CA dashboards. Of procedures with a result, the rate of positive FIT was 79.5% (79.1-79.7). Due to the rates of procedures labelled as FIT not done, no NHS services met the 85% target. When restricting only procedures with a recorded FIT value, 37.1% (27.8-47.5, n=33) of services met this target. Significant variation in recording of FIT values was seen at an endoscopist level.Conclusions FIT data routinely captured by NED allows for real-time reporting of performance with national targets. Data is auditable at multiple levels (endoscopy service, region, integrated care board and CA), and NED can identify outlying endoscopists who under-report FIT values.Work is needed to address two limitations: The variation in FIT reporting by endoscopists and ensuring ubiquitous coverage of endoscopy sites by NEDi2.1.
