Background Community-led organisations (CLOs) are place-based collectives that mobilise people, skills, and physical assets to deliver activities which support health and wellbeing (e.g., gardening, financial advice, creative arts classes). CLOs have a recognised role as third-sector interventions for public health. However, detailed descriptions of the health and wellbeing CLOs generate, and the resources required to inform funding decisions are largely absent.Methods Data from a 12-month longitudinal study of participants from 14 CLOs in the UK, informed a cost-effectiveness analysis (year in full capabilities - YFC), a cost utility analysis (quality adjusted life years - QALY), and a cost consequence analysis (disaggregated costs and benefits). We compared n=331 CLO participants to a ‘do nothing’ synthetic control group (n=99) as there was no suitable real-world comparator. Health and wellbeing measurement was via ICECAP-A capability measure (adults), EuroQol EQ-5D-5L, Short-form Warwick Edinburgh Mental Wellbeing Scale (SWEMWBS) and the Revised Social Connectedness Scale (SCS-R). Resource utilization included CLO, health, social care, and community sector resources.Results Incremental cost per YFC was £35,813. Incremental cost per QALY was £29,827. Statistically significant improvements in SCS-R and SWEMWBS emerged at 12 months in the intervention group. Costs were driven by CLO resource consumption with no significant difference in healthcare resource consumption between groups observed. Costing was challenged by wide variation in CLO activities and organisations’ overheads.Conclusion CLOs confer measurable wellbeing benefits beyond those captured by EQ-5D-5L with a wide range of resource requirements. How public health funding decisions about assets-based approaches may be aided by economic evaluations remains unclear because such decisions do not follow a traditional healthcare technology assessment framework. How and when economic evaluations may be useful in this space and what forms they may take requires further research.
