Triple rule out to make a single diagnosis. But what is it?

A 23-year-old man with a 4-year history of recreational hallucinogenic drug misuse presented with a 3-week history of dyspnoea and lower limb swelling but denied chest pain. He reported no recent travel and medical or family history. He also stated he neither smoked nor drank alcohol.
