E-262 Comparative efficacy of standalone coil versus particle embolization of the middle meningeal artery in chronic subdural hematoma

Introduction/Purpose Chronic subdural hematoma (cSDH) has an incidence of 8.2-14.0 per 100,000 individuals, disproportionately affecting the elderly. Middle meningeal artery embolization (MMAe) has emerged as a minimally invasive technique to reduce vascular supply, promoting hematoma resolution and preventing recurrence. Research into the ideal embolization technique to promote safe, efficacious, and cost-effective treatment is warranted. We aim to evaluate and compare the technical success, safety, and short-term outcomes of standalone coil embolization versus particle embolization in patients undergoing MMAe for SDH.Materials and Methods We completed a retrospective review of 127 patients with a diagnosis of SDH who underwent MMAe with standalone coils or Onyx™ particles at our institution between June 2023 and July 2025. Data collected included demographics, imaging characteristics, procedural details, and outcomes at 1 month, 3 months, and 6 months. Primary efficacy outcome was unplanned repeat surgical drainage; primary safety outcome was major procedural complications.Results A total of 162 embolizations were performed in 127 patients. Of these, 62.0% were treated with standalone coils and 38.0 % with Onyx™ particles. In the coil cohort, the mean age was 71.3 years and 76.6% were male, compared to a mean age of 73.2 years and 80.7% male in the particle cohort. Technical success was achieved in 100% of cases in both groups with a mean follow up duration of 6.75 and 7.7 months, respectively. In the coil cohort, 46.6% of patients with CT follow up at 1 month had hematoma resolution or improvement. Interval resolution or improvement was observed in 22.7% of cases at 3 months and 25.8% at 6 months. In the particle group, 57% of hematomas with CT follow up at 1 month had improvement or resolution. At 3 months, 69% of cases with follow up imaging demonstrated interval improvement or resolution, increasing to 82% at 6 months.Conclusion Standalone coil and Onyx ™ particle embolization for MMAe demonstrated equivalent technical success with no major safety concerns. Both techniques were associated with progressive hematoma improvement, with particle embolization demonstrating improvement more commonly at the earliest interval. This suggests that both methods are safe and effective treatment for the management of SDH. Further studies with longer follow-up are warranted to better define comparative efficacy, procedure length, durability, and cost-effectiveness.Disclosures M. Mitchell: None. M. Morrison: None. S.
