Healthcare quality extends beyond clinical interventions to include appropriateness, safety, timeliness, accessibility, patient experience, and outcomes.1 Audit filters help identify cases that fall outside expected standards, flagging potential deviations or delays. However, the audit filters examined by Al-Khalili et al in this issue of TSACO were limited in scope, leaving important dimensions underexplored.2 Such a narrow focus risks missing critical opportunities for improvement (OFIs) in complex trauma systems shaped by interdisciplinary coordination, resource availability, and institutional culture.2 3 Clarification of the composition and structure of morbidity and mortality conferences at Karolinska would have strengthened the article by indicating whether diverse professional perspectives informed OFI determination.2
