A woman in her 20s presented with concerns of diminution of vision and pain OS(Oculus Sinister) following sudden blast from a firecracker, 1 day prior. On examination, best corrected visual acuity (BCVA) OD (Oculus Dexter)was 6/6 and OS was hand movements close to face. A 3 mm × 4 mm area of scleral ischaemia and melt was noted in the superotemporal quadrant, 3 mm from the limbus in this eye. The lens was clear and vitreous haemorrhage obscuring fundus details was noted. A lamellar scleral patch graft combined with tenonplasty was performed OS. At 3-month follow-up, the globe integrity was well maintained and vitreous haemorrhage had started to clear which revealed superficial retinal haemorrhages near the disc ( figure 1a), giant chorioretinal rupture, resolving retinal and choroidal haemorrhages superotemporally and dense fibroglial proliferation with tractional retinal detachment threatening the macula (figure 1b). Pars plana vitrectomy with release of traction bands and silicone oil tamponade was performed. At the 6-month follow-up, the patient maintains a BCVA of 6/60 OS with attached retina.
