A 66-year-old woman presented to the emergency department with a sudden, sharp epigastric pain that started 1 hour earlier while having dinner. On physical examination, she appeared ill and diffuse abdominal rebound tenderness was evident. The patient was too ill to stand, so a supine chest radiograph ( figure 1) was done.
