Background and Aims Most end-of-life care continues to be provided in the community setting, and thus, there continues to be close working between specialist palliative care services and primary care colleagues. There have been recent changes in the NHS Hampshire and Isle of Wight formulary guidance to include medications with ‘purple’ ratings that GPs can prescribe (that are usually prescribed by specialists in secondary care settings) for emergency, short-term symptom relief for patients in the final days of life receiving palliative care in a community setting. A need has been identified to educate GP and district nursing colleagues who may not be as familiar with the use of these medications via the subcutaneous route for patients who are approaching the terminal stage of their illness.Methods Single-page summary reference guides were developed for six medications – dexamethasone, fentanyl, furosemide, levetiracetam, parecoxib and sodium valproate. Information was collated from the Palliative Care Formulary, British National Formulary, and the local Wessex and Isle of Wight palliative care handbooks. Each guide had sections on indication, dosing, contraindications, cautions, adverse effects, supply, reconstitution and a general note on compatibilities. The guides were reviewed by a hospice consultant, a senior community palliative care nurse and a pharmacist and were approved at a local hospice medicines’ management meeting. The guides were disseminated to local GP practices and district nurses. A survey was conducted before and after the dissemination of the resource to assess education needs, evaluate impact and gain feedback to improve the resource.Results and Conclusion There has been generally positive feedback about the reference guides. There is ongoing work with delivering supplementary teaching sessions to support healthcare professionals to confidently and safely use these medications to provide the best possible care for patients nearing the end of life in the community.
