Ultrasound needle guidance has been the mainstay of regional anesthesia over the last two decades, yet despite its use, the incidence of nerve injury has not fallen. This editorial draws on the paper by Dossi et al 1 in this edition of Regional Anesthesia & Pain Medicine to explain why clinicians encounter difficulties when interpreting images, highlight the physical constraints of standard B-mode ultrasound, and present a novel technology that has the potential to improve the efficacy and safety of nerve block.
