Introduction Inadvertent detachment of stent retrievers during mechanical thrombectomy is a rare but serious complication associated with poor clinical outcomes. We report two such cases involving the pRESET device and evaluate retrieval strategies through mechanical and phantom studies.Aim of study Analyze the management strategies and outcomes following inadvertent detachment of stent retrievers during mechanical thrombectomy, with a focus on the pRESET device, and to evaluate the efficacy of the Tigertriever for retrieval of detached devices through clinical, phantom, and mechanical studies.Method A retrospective review of MT’s performed between 2018 and 2020 at a comprehensive stroke center identified two cases (~0.7%) of unintentional SR detachment. Both patients underwent unsuccessful endovascular retrieval attempts and failed recanalization. Mechanical study was conducted to determine the detachment resistance of various SR’s, and a dedicated vascular phantom study was designed to evaluate the feasibility of retrieving detached pRESET device using the Tigertriever.Abstract A186 Figure 1Results Both patients had persistent large vessel occlusion post-procedure and modified Rankin Scale scores of 3 and 4 at 90 days. In the mechanical tests, the Tigertriever demonstrated the highest resistance to detachment, followed by Embotrap, pRESET, and the 3D Separator. In the phantom model mimicking an M1 segment configuration, Tigertriever successfully retrieved a detached pRESET.Abstract A186 Figure 2Conclusion Conservative management of the inadvertent detachment of stent retrievers during MT in large vessel occlusion may be acceptable in order to avoid further periprocedural complications after unsuccessful device removal attempts.
