In severe exertional heat illness (EHI), inflammation and endotoxaemia may contribute to debility and death.1 The lack of predictive and diagnostic tools reflects a multifactorial aetiology and complex pathophysiology. This challenge is felt acutely in military organisations responsible for service person (SP) exposure to occupational heat stress.2
