P12 Characteristics and outcomes of patients with COPD and acute hypercapnic respiratory failure (AHRF) admitted to respiratory support unit (RSU)

Background Mortality from COPD with AHRF remains high and several prognostic factors have been previously identified.Aim and objectives We aim to demonstrate certain patient characteristics and associated outcomes in COPD patients with AHRF admitted to RSU.Methods We retrospectively collected data of 70 patients with COPD and AHRF admitted to RSU between 1/1/2023 and 1/10/2023. Recorded variables are demographics, clinical data and 3-month outcome. Data was analysed using Excel.Results We classified our patients’ 3-month outcomes into Group I (death, n=19; 27%), Group II (readmission, n=25; 36%) and Group III (no readmission, n=26; 37%) with mean age 75, 68 and 71 respectively. Eosinophilia (Eosinophils ≥0.3 x 10^9/L) was present in 5% of Group I, 27% of Group II and 12% of Group III. Eosinophilia was absent in current smokers, contrary to expectations. 74% of the mortality group had ≥2 comorbidities. 76% (13/17) of patients with FEV1 <30% either died or were readmitted, though most patients lacked FEV1 data. Eosinophilia is not associated with mortality confounded by old age and comorbidities. Male sex, very severe lung function, and eosinophilia correlate with 3-month readmission. Further research is required to identify prognostic factors for treatment optimization.
