A female in her 30s, currently at 34 weeks of gestation, was referred to the medicine outpatient department due to a deranged lipid profile. Although she had a prior history of gestational diabetes mellitus (GDM), her current pregnancy had been uneventful. She was completely asymptomatic, with no current endocrinological or cardiometabolic conditions. Family history revealed diabetes mellitus in both parents, along with an elevated cholesterol level of 1100 mg/dL in her father, for which he was on statin therapy. Clinical examination revealed no xanthomata, hepatosplenomegaly, hypertension or bruits.
