Innovative technologies for asthma and COPD management in the community: scanning the horizon using rapid systematic review methods

Introduction This rapid systematic review and horizon scan aimed to identify technologies available or in development to support the monitoring and self-management of asthma and/or chronic obstructive pulmonary disease (COPD) outside traditional healthcare settings in England.Methods Literature searches targeted systematic reviews from 2020 to 2023 and employed complementary horizon scanning methods to gather data on identified technologies and potentially discover new ones. The Grading of Recommendations, Assessment, Development and Evaluations framework assessed the certainty of evidence for each technology. Sources included medical databases, trial registries, company websites and app stores. Eligible technologies were those used for asthma or COPD, designed for out-of-clinic use, capable of self-monitoring or early exacerbation identification, portable and using artificial intelligence or remote monitoring features.Results A total of 53 technologies were included, comprising 18 digital solutions, 22 devices and 13 prognostic tools, with 22 being readily available and CE marked. Portable spirometers combined with digital applications are promising for alleviating National Health Service backlogs and addressing seasonal pressures while providing care for patients with asthma and COPD. Notably, some technologies may have applications beyond those initially reported by the manufacturer, potentially benefiting different population subgroups.Conclusions This horizon scan underscores the potential of innovative technologies to enhance asthma and COPD management outside traditional healthcare settings.
