A 78-year-old non-smoker man with no relevant medical history presented with a 3-week history of fever, low-volume haemoptysis and asthenia unresponsive to amoxicillin-clavulanate. He had worked as a construction painter and had not travelled outside Europe. On examination, he was febrile (38.2°C) but haemodynamically stable and eupnoeic (SpO₂ 98% on room air).
