A woman in her late 80s with poorly controlled type 2 diabetes (HbA1c 9.0%) presented with a 1 week history of severe pain, swelling and redness in her left second toe, now preventing her from wearing shoes or walking comfortably. She had delayed seeking medical attention, initially trying to manage the pain at home, until worsening swelling and drainage prompted her family to insist she visit the hospital.
