43 1-year follow-up after open re-insertion of the hip abductor tendons on the greater TROchanter shows promising results

Introduction Degenerative lesions of the hip abductor tendons (gluteus minimus and medius) are common but often underdiagnosed and poorly understood. Hvidovre Hospital has been addressing the problem for several years and this study presents our short-term results.Materials and Methods Patients referred with lateral hip pain. Diagnosis was made clinically and confirmed with MRI showing a lesion at the attachment at the greater trochanter. Surgery was performed as an open surgery, with reattachment of the tendon/tendons using suture anchors. Discharged same day with partial weightbearing, followed by 6–12 weeks of specialized rehabilitation. Clinical data, including range of motion (ROM), strength tests, pain assessments, and the patient-reported outcome measures (PROMs): Oxford Hip Score (OHS), Copenhagen Hip and Groin Score (HAGOS), and EQ5D VAS score, were collected at baseline, 3 months, and 12 months post-surgery.Results Twenty-one patients with a mean age of 61.3 years and a mean BMI of 26.3 were followed for one year. Significant improvements were observed: EQ5D increased from 66.5 to 79.3, OHS rose from 23.2 to 38.5, and HAGOS scores improved by an average of 40 points in all items. Pain at rest decreased from 57 to 12.3, and pain during activity decreased from 75.0 to 17.2. Satisfaction and willingness to undergo repeat surgery were both rated 4.7 out of 5.Conclusion The short-term results of open tendon re-insertion surgery are promising, showing significant improvements in pain, function, and patient satisfaction. Continued data collection at 2, 5, and possibly 10 years are needed to evaluate long-term outcomes.
