A 67-year-old man was admitted with involuntary right-sided movements, with episodes of transient unresponsiveness. He had uncontrolled type 2 diabetes mellitus and a previous ischaemic stroke, though was otherwise well, with no significant functional limitations. There was no family history of similar movements. He drank no alcohol and took no illicit drugs.
